Clinic Information and Policies

INSURANCE/PAYMENT:

My policy is for payment to be made at the time of service and upon request, to provide a receipt with all the information insurance companies typically require. Clients may submit their paid receipt to their insurance for direct reimbursement according to their policy. For most insurance companies my services are directly payable under most HSA plans and I am considered an out-of-network provider.

BILLING:

I have a small specialized practice that cannot carry balances and does not have a billing system.

RATES:

Single 45 minute session $200

Single 45 minute court related session $225

Single 60 minute session $250

Double Session 1 Hour, 30 minute session $400

Double Session 1 Hour, 30 minute court related session $450

prorated rates applicable to extended sessions

Court/Legal Hearing/Subpoenaed appearances $350/hour with a

$1,000.00 minimum paid in advance, including preparation/travel time.

PAYMENT METHODS:

CREDIT Card subject to 3.5% service charge

DEBIT Card subject to 3.5% service charge

CHECK payable to Farmington Hills Counseling Services (F.H.C.S.)

Covid policy: April 2023

In an ongoing effort to minimize exposure to the Coronavirus for the safety of my patients and my family I am utilizing Zoom, Telephone, FaceTime sessions, and in-office appointments as requested, or for more urgent or emergency sessions.

I will continue to assess this unfolding situation and amend these practices as necessary.
